Antibodies: Pointing out Pathways in Pathology

The importance of antibodies in diagnostic pathology has been significant for some time since the introduction of immunohistochemistry, flow cytometry, and clinical immunoassays. Thus far, approximately 350 cell surface antigens alone have been described with corresponding monoclonal antibodies in the cluster of differentiation/designation (CD) system of nomenclature. In addition, knowledge gained in the genetic pathways of tumorigenesis and other disease processes has further expanded the role of antibodies in molecular profiling and prognostication via such methodologies as multiplex tissue microarray. These advances have also led to new ethical issues involving patient care and consent as antibodies can, in some cases, directly point to the likely presence of an underlying genetic defect (e.g. germline defects in DNA mismatch repair enzymes, MSH2, MSH6).

Molecular properties that are critical in determining antibody efficacy include sensitivity and specificity, avidity, level of variability from lot to lot, and structural stability. Other considerations include production costs and clinical applicability. One emerging and exciting area of antibody research involves the development of single-domain antibodies (Nanobody; Ablynx). These (small) molecules are actually monomeric antibody fragments composed of the variable domain, and have increased cellular permeability. This property may further magnify the role of antibodies in pathology as indicators of altered intracellular signaling in various disease states.
