Laboratories: Cornerstone of Pathology Practice

Pathology laboratories provide the basic platform for the delivery of pathology services in both anatomic and clinical pathology. Most laboratories operate either within a hospital or in a private community setting. Reference and research laboratories are especially suited for conducting complex or esoteric tests that require specialized instrumentation (e.g. flow cytometry) and/or expertise in a subspecialty field (e.g. molecular genetics, hematopathology).
Reference laboratories (e.g. ARUP, Mayo Medical Laboratories, Quest Diagnostics, etc.) play a significant role in the delivery of pathology services as the number and complexity of available tests continue to increase. Current economic forces within the health care industry have prompted referring laboratories to carefully analyze test volume, third-party reimbursement, and reference laboratory utilization reports in order to make cost-effective decisions regarding send-out versus in-house testing. Potential concerns of send-out testing for referring laboratories include turn-around times, remote access to laboratory results, and accuracy and standardization of CPT (current procedural terminology) coding. One potential barrier that reference laboratories may face is restricted access to patient clinical histories either due to HIPAA (Health Insurance Portability and Accountability Act) regulations and/or lack of interface capabilities with the referring institution. These issues underline the importance of direct communication among the reference laboratory, referring laboratory, and clinicians in optimizing patient health care.
