Pathology Education and Pharmacogenomics: At the Crossroads

Pathologists have often been called "the doctors' doctor". This expression is becoming particularly salient in the era of personalized medicine as an increasing number of molecular diagnostic tests are being utilized in guiding therapeutic decisions in areas such as infectious disease and oncology. While training in molecular genetics has already been integrated into pathology residency (and fellowship) programs across the country for some time, the emphasis has been mainly on pathophysiology and diagnosis. As more targeted therapeutic drugs (e.g. monoclonal antibody based therapies) are being approved by the FDA and other agencies internationally, pathologists will be expected to develop a familiarity with the genetic basis of drug therapy in addition to the regulatory aspects of in vitro diagnostic testing.
Pharmacogenomics has already made its foray into the vast and diverse specialty of pathology. For example, several pharmacogenomics online courses and case studies for CME/SAM credits are currently being offered by the College of American Pathologists (CAP). In addition, a number of abstracts presented at meetings have focused on translational research projects involving the interaction of genetic signatures (e.g. molecular profiling of tumors) with drug efficacy. If this trend continues, the evolving field of pathology may even incorporate pharmacogenomics as a formal subspecialty in the future.
