Pathology Grossing Tools of the Trade: Blades and Scalpels

Microscopic histopathologic examination of tissue is just one of several steps in achieving an
accurate diagnosis in pathology. Achieving a correct diagnosis starts at the grossing bench, assuming the
specimen is in an intact condition. Grossing involves careful examination and representative sectioning of
the specimen for generating microscopic slides. While one needs good eyes for visual inspection, good
cutting tools are needed to optimize the quality of sections while ensuring the safety of the person grossing.
Specimen integrity is essential for high quality microscopy; blunt, ill-fitting, or damaged blades
(e.g. tears in tissue caused by nicked blades) can cause tissue artifacts. Sharps injuries are also an important
issue. To mitigate these problems, vendors offer innovative designs for many types of surgical and
postmortem specimens. For example, the disposable scalpel can be an effective solution for preventing
sharps injuries. Some scalpel handles for disposable blades have corrugated edges to prevent slippage,
especially with fatty or bloody specimens. Angulated microsurgical scalpels are ideal for the fine
dissection of small biopsies (Electron Microscopy Sciences). In addition, there are accessory tools for
specimen immobilization and consistent sectioning to optimize the standardization of the grossing
