Definition / general
  • Also called spindle cell pseudotumor, plasma cell granuloma, inflammatory myofibroblastic tumor
  • Rare, nonneoplastic lesion
  • May be intracranial extension of extracranial tumor
  • Usually in lung and airways
  • Rarely intracranial or within spinal cord affecting young men
Case reports
Treatment
  • Complete resection, variable radiation therapy or steroids
  • May recur
Microscopic (histologic) description
  • Inflammatory proliferation of polyclonal plasma cells with variable lymphocytes, neutrophils, eosinophils and histiocytes
  • Fibrovascular background
  • Variable giant cells, calcifications, circular fibrosis of small veins

Immunohistochemistry
  • Plasma cells are polyclonal
  • Acid fast bacilli if due to mycobacteria
Differential diagnosis
  • Lymphoma: monoclonal
  • Lymphoplasmacyte rich meningioma: has areas of classic meningioma, EMA+
  • Plasmacytoma
