Definition / general
  • Distinct clinicopathologic entity characterized by direct derivation from low grade dysplasia mucosa of inflammatory bowel disease (Am J Surg Pathol 2006;30:1022)
  • Very well differentiated morphology
  • Development of these tumors supports recommendation for aggressive management of IBD patients with LGD
Microscopic (histologic) description
  • Very well differentiated small to medium diameter glands with round or tubular profiles, low grade cytologic characteristics, no/rare desmoplastic reaction
  • Invasive glands usually resemble overlying low grade or indefinite dysplastic crypts
Positive stains
  • Frequent coexpression of CK7 and CK20
Negative stains
  • Frequent silencing of hMLH1
