- Capsular or subcapsular involvement with 20-30 normal appearing colloid filled thyroid follicles
- Metastatic papillary carcinoma: assume metastatic unless see small aggregate of follicles that are round/oval, loosely arranged with no papillae, normal sized nuclei and nucleoli, fine chromatin, no crowded nuclei, no stromal reaction, no psammoma bodies
