Definition / General
Epidemiology
  • Usually premenopausal women
  • Outside ovary, common sites are head and neck, deep soft tissue
Etiology
Clinical Features
  • Abdominal pain and distension
Laboratory
  • Tumor markers CA-125, carcinoembryonic antigen, β-human chorionic gonadotropin, and α-fetoprotein are mostly normal (J Clin Oncol 2010;28:e675)
Radiology Description
  • Ultrasonography and computed tomography reveal heterogeneous adnexal mass

Treatment
  • Radical surgical excision followed by various chemotherapeutic regimens
Gross Description
  • Unilateral mostly in right ovary, hemorrhagic, median 13 cm
  • Solid cystic with smooth to bosselated surface

Micro Description
  • Anastomosing vascular spaces with slit-like lumina interspersed with solid fascicular areas
  • Pleomorphic, epithelioid to focally spindled cells, few with intracytoplasmic lumina
  • Brisk mitotic activity, necrosis, hemorrhage
  • Vascular nature often not immediately apparent, but at least focal vasoformative channels or discrete cytoplasmic vacuoles
  • Variable features: fascicular growth pattern composed of spindle cells with ovoid nuclei and ample eosinophilic cytoplasm mimicking leiomyosarcoma; reticular growth pattern resembling yolk sac tumor; features resembling clear cell carcinoma
