Definition / general
- Diffusely infiltrating astrocytoma negative for IDH mutations by immunohistochemistry (for mutant IDH1-R132H protein) and sequencing (for IDH1 codon 132 and IDH2 codon 172 gene mutations)
- Most likely constitutes a variety of other entities with different clinical courses
- Entities which have been found to be incorrectly diagnosed as diffuse astrocytoma IDH-wildtype include:
- Glioblastoma, conventional type or H3F3A mutated
- Diffuse midline gliomas with histone H3‐K27M mutation
- Pilocytic astrocytoma
- Normal or reactive brain tissue (J Neuropathol Exp Neurol 2018;77:422, Acta Neuropathol 2015;130:407)
- Diagnosis should be used with caution (Acta Neuropathol 2015;130:407)
- Often lacks mutations in p53 and ATRX
- Many IDH1 / 2 wildtype adult diffuse gliomas behave adversely, similar to glioblastoma (Acta Neuropathol 2015;130:407)
